Why do accident investigations still so often end up focusing on errors and shortcomings, even when we aim for a systemic perspective? Join Grete Horsbøl from the Danish Maritime Accident Investigation Board about the challenges of turning systemic investigation theory into meaningful practice.

About the event

What?

As a way to dive a little deeper into the more practical ways of conducting incident/accident investigations, we are pleased to welcome Grete Horsbøl from the Danish Maritime Accident Investigation Board (DMAIB) !

“Systemic” has become a buzzword in safety work. It reflects a real appetite for deeper explanations of accidents: explanations that go beyond pointing to errors and deficiencies, show how the wider system shapes what people do, and capture the complexity of operations and organisations.

The intentions are good. Yet many accident reports look much as they always have. They explain accidents by evaluating human actions, training, equipment, constructions, up against procedures and regulations. Accidents are understood as deviations from the idea of how the system should work. Measured against that ideal, the accident once again becomes a story of errors and deficiencies. The explanation turns black and white: right or wrong, good or bad. What gets lost are the nuances, the different perspectives, the shifts and the frictions that make up the complexity of real operations.

In this webinar, Grete Horsbøl from the Danish Maritime Accident Investigation Board (DMAIB) looks at what happens when systemic investigation moves from theory to practice. Why is it so hard to map and understand a system rather than judge it? And what would it take for reports to explain accidents through the system instead of through its shortcomings?

The webinar will be held in English and be recorded and later published at the SÄKU Member Page. As always, you as participants will have the chance to ask your questions live.

About Grete Horsbøl

Grete Horsbøl Horsbøl has been a marine accident investigator at the Danish Maritime Accident Investigation Board since 2013. She specialises in accident analysis, the development of investigation methods and report writing. With an MA in Comparative Literature, she is particularly interested in the cultural context of accident investigation: how our understanding of accidents is shaped by history and culture, and how language reflects that understanding.

About DMIAB (Danish Maritime Accident Investigation Board)

The Danish Maritime Accident Investigation Board (DMAIB) is an independent body under the Danish Ministry of Industry, Business and Financial Affairs. It investigates accidents on Danish and Greenlandic ships, accidents on foreign ships in Danish and Greenlandic waters, and serious accidents in commercial diving.

The aim of each investigation is to establish what happened and why, and to share new knowledge that can improve safety at sea. DMAIB does not apportion blame or liability.
